Are occupational therapists essential for rehabilitation and recovery?

Occupational therapists (OTs) embrace a holistic approach to support individuals of all ages in reclaiming their personal roles and lifestyles, regardless of injuries or physical illnesses. Collaborating closely with physiotherapists, OTs ensure that treatment modalities complement one another. While physiotherapy strengthens the body, occupational therapy empowers individuals to put on their dancing shoes and fully engage in life.

Why occupational therapy matters
Occupational therapy services encompass comprehensive care plans that consider a client’s personal profile and psychological aspects, including their home, school, college, and community environments. This holistic perspective focuses on therapeutic activities, daily living skills, independent care, motor relearning, neurodevelopmental strategies, and continuity of home exercises.

What to expect from occupational therapy
As assessors, OTs identify each client's unique needs and potential, tailoring their approach based on individual goals. The process is client-centered, emphasizing discussions about what patients aspire to achieve in order to enhance their quality of life.

Dania (pix), an occupational therapist at Sunway Healthcare, explains, “Most of the time, patients express desires like wanting to drive again, return to work, or ride a bike. Based on these expectations, we develop client-specific activities and training plans that align with their goals. A comprehensive care plan is crafted following thorough assessments.” 

She was speaking on “Reclaiming your health: How physiotherapy and occupational therapy can transform your life” at The Art in Orthopedic Care & Management - Orthopedic Symposium by Sunway Medical Centre, Sunway City, Kuala Lumpur.

Assessments and caregiver education
OTs conduct a variety of assessments, including handwriting, reading, listening, driving, and cognitive evaluations. They also provide caregiver education to prepare families for supporting patients at home. Home visits are conducted to ensure a safe living environment, minimizing fall risks, while school visits help identify challenges faced by children post-surgery or with disabilities.

How occupational therapy works
Referrals and evaluations are essential to establishing a client-centered care plan. The implementation of this plan varies based on individual goals:
  • Pre-surgery to post-surgery care: Rehabilitation frequency is determined by specific goals, typically ranging from 6 to 10 hours per week.
  • Inpatient to outpatient care: Rehabilitation hours may vary from 6 to 14 hours per week for intensive recovery lasting one to three months.
  • Conventional to integrated robotic approaches: The frequency of conventional and robotic therapies is tailored after assessing motor relearning needs.

Collaboration with physiotherapists
Occupational therapists and physiotherapists share similar treatment objectives, with their approaches complementing one another to enhance client recovery. While physiotherapists focus on improving strength, balance, and coordination, OTs emphasize helping clients perform daily tasks, engage in social interactions, and reintegrate into their communities.

Specializations in occupational therapy
Occupational therapy encompasses various specializations, including:
  • Upper extremities: Splinting, pain management, functional hand training, and advanced upper limb robotics.
  • Amputee care: Stump management, pre- and post-prosthetic training, and sensory re-education.
  • Ergonomics: Assessments and recommendations for workplace ergonomics.
  • Burn and scar management: Pressure garments and scar management techniques.
  • Spinal care: Caregiver education, pre- and post-surgical management, and support for returning to work or school.
  • Pain management techniques: Pain management strategies include relaxation techniques such as deep breathing, progressive muscle relaxation, and visualization or graded imagery.

Occupational therapists play a crucial role in the rehabilitation process, helping individuals not only recover physically but also regain their sense of identity and purpose in life. Through their comprehensive and personalized approach, OTs empower clients to thrive in their daily activities, fostering independence and enhancing overall well-being.
