​Hydrotherapy: A promising path to stroke rehabilitation

Stroke, or cerebrovascular disease, ranks as Malaysia's third leading cause of death, with alarming statistics from 2019 showing 47,911 incident cases, 19,928 deaths, and 443,995 prevalent cases. Additionally, a staggering 512,726 Disability-Adjusted Life Years (DALYs) were lost due to stroke. The repercussions of a stroke extend beyond immediate health concerns, leading to chronic impairments that severely limit an individual’s ability to perform daily activities.

The need for effective rehabilitation
Following a stroke, many individuals engage in multidisciplinary rehabilitation programs designed to aid recovery. However, despite the comprehensive nature of these rehabilitation efforts, research indicates that only about 5% to 20% of stroke patients achieve complete functional recovery (Nakayama, 1994). This statistic highlights an urgent need for innovative inpatient and outpatient rehabilitation strategies tailored to meet the unique needs of stroke survivors and their caregivers (Barker, 2005).

Exploring hydrotherapy as a rehabilitation approach
Among the various rehabilitation methods available, hydrotherapy—also known as water-based exercises—stands out as a promising option. Hydrotherapy involves a structured therapy program utilizing the unique properties of water, designed by qualified physiotherapists to enhance physical function. Ideally, these exercises take place in specially designed, heated hydrotherapy pools that provide an optimal environment for rehabilitation.

Dr Rishiken Kuppusamy (pix), a consultant neurologist at Pantai Hospital Kuala Lumpur, notes, “Apart from various specific techniques, simple exercises such as general body movements and walking in water might be beneficial because water's natural buoyancy allows many body movements by providing a type of body weight support. The natural resistance of water may encourage strengthening of weakened muscles.” This dual benefit of buoyancy and resistance makes hydrotherapy particularly effective for stroke patients, who often struggle with muscle weakness and mobility issues.

Historical context and evolving interest
The interest in hydrotherapy can be traced back to its applications in rehabilitating individuals affected by poliomyelitis several decades ago. This historical context has shaped the focus on hydrotherapy as a viable option for those experiencing muscle weakness. Recent studies have shown that water-based exercises can significantly improve fitness and strength in older adults and individuals with conditions such as rheumatoid arthritis.

Potential benefits of hydrotherapy for stroke patients
Hydrotherapy presents several potential benefits for stroke patients, including:
  • Improved Activities of Daily Living (ADLs): Engaging in water-based exercises can enhance the ability to perform daily tasks, promoting independence and improving quality of life.
  • Enhanced cardiovascular fitness: Hydrotherapy may also contribute to improved cardiovascular health, which is crucial for overall recovery and well-being post-stroke.
  • Strengthening weak muscles: The resistance offered by water can help strengthen muscles that have been weakened due to the stroke, facilitating better mobility and functionality.
  • Psychological benefits: The soothing nature of water can provide psychological comfort and reduce anxiety, making the rehabilitation process more enjoyable for patients.

The need for systematic evaluation
Despite the promising advantages of hydrotherapy, it is essential to note that a systematic evaluation of its effectiveness for stroke patients is still lacking. More research is needed to establish standardized protocols and guidelines that can optimize the use of hydrotherapy in stroke rehabilitation.
