At the recent Aesthetic Medicine & Surgery Conference & Exhibition (AMSC), Dr Tom Van Eijk (pix), one of Holland’s most famous cosmetic doctors and a global authority on injectable fillers, spilled the tea on lip tenting—a technique he pioneered back in 2005. Lip tenting: What’s the goal? The objective of lip tenting isn’t just about making lips bigger (although that’s a nice perk). No, we’re also looking to elongate the vermilion—aka, the red part of your lips—because, let’s face it, we all love a juicy pout. Plus, this technique helps restore the "white roll" around your lips that often falls victim to aging and sun damage. A refreshed white roll adds structure and a clean contour. So not only are you volumizing, but you're also getting a subtle shape enhancement. Lip migration: Not a new reality show But wait—there’s more! The gel used in lip tenting needs to stay put. The dreaded "migration" occurs when the filler decides to take a little vacation and drift off into the wrong tissues. How do you avoid that? Well, you start with a gooey, soft hyaluronic acid gel that's more prone to spreading (think: a slippery jelly). When this happens in areas with weaker connective tissue, you lose control faster than trying to hold onto a greased watermelon. A cannula (the larger, blunt-tipped tool often used in lip procedures) is excellent for maneuvering in these tricky tissues. But be warned, the more it can move, the more your gel will say, "I'm outta here!" Think of it as trying to squeeze too much toothpaste onto your brush—it’ll go everywhere but where you need it. Stopping the slide: Tighter gel, tighter control To keep that filler where it belongs, opt for sturdier hyaluronic acid in denser tissue. Smaller quantities and lower pressure are key. As Dr Tom puts it: "It’s better to inject a bunch of grapes than a melon." So, tiny injections spread out across your lip will prevent the dreaded filler slide. And trust the needle here. Why lip tenting is a needle technique Lip tenting relies on a needle for a reason. This isn’t a cannula show; it’s all about precision. Each stroke of the needle penetrates through different tissue layers, providing varied effects. In cross-section, the filler strengthens the tissue, while also elongating the vermilion to create that fuller, more youthful look. The technique involves injecting at a 45-degree angle from the oral hemisphere (that’s doctor-speak for "inside of your lip") towards the middle of your lips, ensuring volume and shape enhancements. On the lower lip, the filler gives that much-desired curvature as the vermilion elongates, and the white roll moves slightly closer to the nose, making your lips look naturally lifted. Gel selection: Small particles or big decisions? Dr Tom gives some sage advice: When it comes to choosing your hyaluronic acid gel, consider the lip as a reconstructive area. Small, firm particles for structure and strength, while larger particles give more lasting volume. The key to success? Focus on the superficial layers and place those injections close together to avoid unsightly gaps.
