Lupus and pregnancy: Navigating challenges and embracing motherhood

​​​For many women diagnosed with lupus, the thought of pregnancy can be daunting. Lupus, or systemic lupus erythematosus (SLE), is a chronic autoimmune disease that predominantly affects women during their reproductive years, between the ages of 15 and 45. The unpredictability of lupus flare-ups and potential complications may seem overwhelming. However, with the right guidance and management, women with lupus can successfully plan for a safe and healthy pregnancy.

A complex relationship: Lupus and reproductive health
Lupus can pose significant challenges during pregnancy, as it may cause menstrual irregularities and conditions that can lead to infertility. Despite these concerns, many women with lupus can conceive and deliver healthy babies. According to Professor Dr Rozita Mohd (pix), Senior Consultant Physician and Nephrologist at Hospital Tuanku Muhriz Universiti Kebangsaan Malaysia (HCTM UKM), pregnancy is feasible when lupus is inactive and well-controlled.

Preparing for pregnancy with lupus
Close monitoring and thorough planning are essential. Ideally, pregnancy should be considered only when lupus has been inactive for at least six months. Key health markers include maintaining a urine protein creatinine ratio (UPCR) of less than 50 mg/mmol and a glomerular filtration rate (GFR) above 50 ml/min.

Before conception, patients should switch to a "pregnancy-friendly" medication regimen for at least three months. Safe medications include hydroxychloroquine, low-dose prednisone, azathioprine, and calcineurin inhibitors (CNI). Importantly, the intensity of treatment should not be reduced solely because of pregnancy plans, as maintaining disease stability is crucial.

Medications to avoid
Some medications, like mycophenolate mofetil (MMF), are strictly contraindicated during pregnancy due to teratogenic effects. Aspirin may be recommended to reduce the risk of pre-eclampsia, a condition linked with lupus flare-ups.

Potential risks during pregnancy
Pregnancy can trigger lupus flare-ups and lead to complications such as hypertension, pre-eclampsia, and lupus nephritis, which can be life-threatening. Statistically, maternal mortality in lupus patients is approximately 20 times higher than in the general population. Moreover, fetal complications include intrauterine growth restriction, spontaneous abortion, premature delivery, and, in rare cases, neonatal lupus.

Managing lupus flares during pregnancy
Hydroxychloroquine remains the first-line treatment during pregnancy due to its safety profile. Azathioprine, at a dose of 2 mg/kg per day, can be considered if necessary. However, medications like cyclophosphamide, rituximab, and methotrexate should be strictly avoided.

The role of anti-phospholipid antibody syndrome
Women with lupus are at risk of developing antiphospholipid antibody syndrome (APS), which can severely impact pregnancy outcomes. Lupus anticoagulant is linked with higher risks, but early treatment can significantly improve the live birth rate by up to 85%.

Planning for the best outcomes
Early evaluation and strategic planning are crucial. Pregnancy should be considered only if there has been no lupus flare for at least six months prior to conception. Following professional medical advice and adhering to medication protocols can greatly enhance the chances of a successful pregnancy.

Living with lupus does not mean giving up on the dream of motherhood. With careful planning, medical guidance, and a proactive approach to managing the condition, many women with lupus can have safe pregnancies and healthy babies.
