​Stroke: Brain tissue death and the importance of timely treatment

A stroke is a medical emergency characterized by the death of brain tissue due to an interruption of blood flow to the brain. This interruption can occur in two primary ways:

Ischemic stroke: This is the most common type, accounting for approximately 79.4% of all stroke cases. It occurs when a blood vessel supplying blood to the brain is blocked, often by a blood clot.

Hemorrhagic stroke: This type accounts for about 18.2% of cases and occurs when a blood vessel in the brain ruptures, causing bleeding in or around the brain.

Additionally, there are transient ischemic attacks (TIAs), which are temporary interruptions in blood flow that typically resolve within 10 - 15 minutes. While TIAs are not strokes, they serve as critical warning signs of potential future strokes.

Stroke statistics in Malaysia
According to the Department of Statistics, Malaysia, stroke has emerged as one of the top five leading causes of mortality since 2000. Data from 2017 indicated that cerebrovascular diseases, which include strokes, contributed to 7.1% of all recorded mortalities in the population.

The mean age for experiencing an ischemic stroke in Malaysia is around 62.8 years for the first occurrence and 64.3 years for recurrent cases. Alarmingly, studies have shown that Malaysian women tend to experience more severe strokes, have a higher number of risk factors, and poorer functional outcomes compared to men. Interestingly, post-gender age adjustment reveals an increased incidence of stroke in women, despite no differences in access to thrombolysis treatment between genders.

Recognizing the symptoms
Dr Rishiken Kuppusamy (pix), a consultant neurologist at Pantai Hospital Kuala Lumpur, emphasizes that strokes typically present with sudden, one-sided symptoms. These can include:
  • One-sided paralysis
  • Numbness on one side of the body
  • Facial drooping on one side
  • Visual disturbances
  • Drowsiness or confusion
  • Vomiting

These symptoms may manifest in isolation or in combination, and it's crucial to seek medical attention immediately if you or someone else experiences them.

The golden window for treatment
Time is of the essence when it comes to stroke treatment. The "golden window" for effective intervention is typically within three to four and a half hours of the onset of symptoms. During this time, medical professionals can administer treatments that may reverse the effects of a stroke, restoring blood flow and minimizing damage to brain tissue. After this window closes, the chances of reversing symptoms decrease significantly.

Understanding risk factors
Several risk factors contribute to the likelihood of experiencing a stroke, many of which are similar to those for heart disease. These include:
  • Age: The risk of stroke increases with age, particularly for individuals over 50.
  • Lifestyle choices: Smoking, excessive alcohol consumption, and drug abuse (e.g., stimulants like ecstasy) are significant contributors to stroke risk.
  • Diet: A diet high in processed foods can lead to conditions like obesity, diabetes, and hypertension, all of which increase stroke risk.
  • Medical history: Conditions such as diabetes, hypertension, and heart disease can predispose individuals to stroke.

In Malaysia, the demographic breakdown of stroke patients reflects the country's population, predominantly affecting Malays, Chinese, and Indians. The underlying causes of strokes often mirror those that lead to heart attacks, as the same vascular issues affect both the heart and the brain.

The role of healthcare professionals
Doctors utilize special scoring systems to assess an individual’s risk factors based on age, smoking habits, presence of diabetes, hypertension, and cardiac issues. This assessment can help identify those at higher risk and initiate preventive measures.

Prevention and awareness
Recognizing the signs of stroke and understanding risk factors are crucial steps in prevention. Here are some proactive measures you can take:
  • Maintain a healthy lifestyle: Adopt a balanced diet rich in fruits, vegetables, whole grains, and lean proteins. Limit processed foods high in sugars and unhealthy fats.
  • Stay active: Regular physical activity can help manage weight and reduce the risk of chronic conditions associated with stroke.
  • Quit smoking: If you smoke, seek resources to help you quit. Smoking significantly increases your risk of stroke and other cardiovascular diseases.
  • Monitor health: Regular check-ups can help manage conditions like hypertension and diabetes, which are critical in stroke prevention.
