The complex landscape of drug abuse in Malaysia: A multifaceted analysis

The issue of substance misuse in Malaysia has a long and complex history, with various factors contributing to its prevalence. The consumption of opioids, including heroin, morphine, opium, oxycodone, hydrocodone, and codeine, has had significant effects on individuals and communities. These substances can be consumed through injection, ingestion, smoking, or snorting, leading to a range of effects such as pain relief, euphoria, and drowsiness. However, the consequences of opioid abuse can be severe, including nausea, constipation, confusion, sedation, respiratory depression, unconsciousness, coma, and even death.

Historical context and policy responses
'The historical context of drug abuse in Malaysia is multifaceted. In the pre-dependence era, opium was the primary substance abused by Chinese immigrants. However, in the post-dependence era, particularly starting in the 1960s, the influence of hippie culture led to an increase in drug abuse, with a greater number of Malay individuals involved. This period also saw the establishment of 'the Golden Triangle' in Burma, Laos, and Thailand, which significantly boosted the drug supply,' ​Dr Anne Yee, Addiction psychiatrist, KPJ Puteri said during an online seminar organized by Monash University Malaysia Clinical School Johor Bahru.

In response to the escalating issue, Malaysia implemented a zero-tolerance policy. By 1983, the number of identified addicts had surged to 92,310, prompting the Prime Minister to declare drugs as the nation's primary adversary and a threat to national security. The Drug Dependent Act was enacted, mandating the compulsory detention of drug users in 'Pusat Serenti' for up to two years to achieve complete abstinence from drugs. Despite the government's substantial expenditure on rehabilitation efforts, the results were disheartening, with a relapse rate of up to 90% and a surge in criminal offenses and infectious diseases such as hepatitis B and C.

Harm reduction initiatives and ongoing challenges
One of the most devastating consequences of drug abuse was the dramatic increase in HIV cases, particularly among drug addicts using needles. This surge in HIV cases prompted the government to reassess its approach, leading to the initiation of a harm reduction program in 2005. This program aimed to address the HIV epidemic among drug users and included initiatives such as the MAT program and the Malaysian Needle Syringe Exchange Program (NSEP).

The statistics on drug abuse in Malaysia reveal a concerning trend, with opiate and marijuana abuse ranking prominently. The number of drug addicts has continued to rise, with a significant increase in amphetamine abuse. The prevalence of drug abuse varies across different states, with factors such as peer pressure, curiosity, stress, and other influences contributing to the issue.

The experience of drug use is influenced by various factors, including the route of administration, effects, purity, potency, quality, form, price, availability, interactions, individual characteristics, and environmental influences. These multifaceted elements contribute to the complexity of addressing drug abuse in Malaysia and underscore the need for comprehensive and targeted interventions to combat this pressing public health concern.

In conclusion, the landscape of drug abuse in Malaysia is intricate and multifaceted, shaped by historical, social, and policy-related factors. While efforts have been made to address the issue through stringent policies and harm reduction initiatives, the challenges persist, with the prevalence of drug abuse continuing to pose significant public health concerns. Addressing this complex issue requires a comprehensive approach that considers the historical context, policy responses, and ongoing challenges to effectively combat drug abuse in Malaysia.
