Datuk Dr Kuljit Singh, President of the Association of Private Hospitals Malaysia (APHM), delineates six key areas driving this transformation: value-based healthcare, telehealth and virtual care, integrated care arrangements, specialized services, personalized medicine, and data management, along with the pivotal role of private insurers and clinics.
Value-based healthcare: Shifting the paradigm Value-based healthcare shifts the focus from service volume to patient outcomes, incentivizing healthcare providers to prioritize preventive care, coordination, and patient satisfaction. This approach is increasingly embraced by private insurers and clinics as a means to enhance patient well-being and cost-effectiveness. "When discussing value-based healthcare, it's important to acknowledge that healthcare costs aren't decreasing; rather, they continue to rise. However, in today's healthcare landscape, the emphasis lies on assessing the rewards offered by healthcare providers in relation to the costs incurred. Prioritizing preventive care, coordination, and patient satisfaction becomes paramount. Datuk Dr Kuljit further highlighted that Prince Court Medical Centre, a facility owned by IHH, procedures such as total knee replacement, anterior cruciate ligament arthroscopic surgery, percutaneous coronary intervention, colonoscopy, hysterectomy, and laparoscopic cholecystectomy are meticulously evaluated - to measure the value delivered to patients and assess the outcomes they achieve during their hospital stay. "We utilize the optimal care index to gauge patient outcomes across the six procedures conducted at our hospital. By analyzing data and sharing outcomes, we demonstrate the value our patients receive. Quality surpasses cost considerations; what matters most are the clinical outcomes and patient experience. As we look to the future of healthcare, these value-driven outcomes exemplify the approach taken at IHH. "These are the diverse value-driven outcomes we explore, allowing us to demonstrate to our patients the value our procedures provide in comparison to other healthcare facilities. Telehealth and virtual care: Bridging gaps in access Telehealth and virtual care revolutionize healthcare access by enabling patients to remotely engage with healthcare services through video conferencing or digital platforms. This approach is especially advantageous for individuals residing in remote or underserved regions, as well as those facing challenges in physically accessing healthcare facilities. Moreover, telehealth and virtual care play a pivotal role in enhancing care coordination and facilitating seamless communication among healthcare providers. In the context of telehealth, Malaysia stands out as a leader, boasting a mature infrastructure well before the global surge in telehealth utilization during the pandemic. Accessing healthcare services is relatively convenient, often without the need for digital logins, particularly when nearby doctors are readily available. However, the significance of telehealth shines through, particularly for remote consultations or follow-ups, where physical hospital visits are impractical. Hence, investing in telehealth and virtual care presents a compelling opportunity for business growth and service enhancement. Datuk Dr Kuljit pointed out that telehealth's relevance in Malaysia dates back to 1997 when Tan Sri Dato Dr Abu Bakar Sulaiman, the Director General of Health, championed its potential. Despite initial skepticism, the pandemic of 2020 highlighted the invaluable role of telehealth, prompting numerous hospitals to explore its integration into their healthcare delivery systems. Patients now have the flexibility to choose between in-person consultations and virtual appointments, leveraging the versatile telehealth platform. Integrated care arrangements Integrated care arrangements entail collaborative efforts among healthcare providers to deliver comprehensive care to patients. This model emphasizes care coordination and communication among providers, proving particularly beneficial for patients with complex medical needs. By minimizing duplication of services and enhancing communication between providers, integrated care arrangements have the potential to enhance patient outcomes while simultaneously reducing costs. "Hospitals traditionally operate in silos, leading to inefficiencies when patients transition between different healthcare facilities. This disjointed approach results in redundant procedures and fragmented care. Recognizing the importance of integrated care, stakeholders must move beyond isolated hospital-to-hospital collaborations. Integration should extend across public and private sectors, involving pharmacies, allied health professionals, and other healthcare sectors to foster seamless coordination. "Integrated care models not only improve outcomes but also yield cost savings. Following the pandemic, there has been a notable emphasis on public-private partnerships in Malaysia, as evidenced by the commitment of ministers such as Dr Zaleha Mustafa and Datuk Seri Dr Dzulkefly Ahmad. Collaborating allows for resource optimization, leveraging the strengths of both sectors. Datuk Dr Kuljit further pointed to the scenario in the Klang Valley, where private hospitals possess numerous CT scan machines, often underutilized after regular hours. Conversely, government hospitals face excessive wait times due to overuse of their CT scan machines. Collaborative partnerships enable efficient resource allocation, alleviating patient burdens without the need for additional public hospital infrastructure. "The seamless collaboration between public and private sectors, highlighted during the pandemic, underscores the importance of unified efforts in healthcare delivery. While past attempts at collaboration may have been hindered by bureaucracy, the urgency of the pandemic demonstrated the potential for swift and effective cooperation. Moving forward, fostering such collaborative initiatives remains imperative for a resilient healthcare system. Specialized services at scale: Meeting unique healthcare needs Specialized services at scale involve the provision of highly specialized healthcare services to a large number of patients. This model enables healthcare providers to deliver high-quality care more efficiently and cost-effectively, particularly benefiting patients with rare diseases or conditions requiring specialized treatment. Emerging examples of specialized private healthcare include Hospital Picaso, Cardiac Vascular Sentral (CVS) Kuala Lumpur, ALTY Orthopaedic Hospital, and KL Fertility Centre. Datuk Dr Kuljit emphasized that the healthcare landscape is evolving towards specialized services, diverging from the traditional model where hospitals offered a wide range of medical specialties under one roof. While the comprehensive approach has its advantages, recent trends indicate a shift towards hospitals focusing on specific medical disciplines. While it's impractical to have a specialist hospital for every medical specialty, the emergence of specialized hospitals demonstrates a growing preference among patients for tailored treatment in facilities dedicated to their specific needs. Personalized medicine and data management: Harnessing the power of AI Personalized medicine and data management, driven by artificial intelligence (AI), revolutionize healthcare delivery by tailoring medical treatments to individual patients based on their unique genetic, environmental, and lifestyle factors. Effective data management is essential for implementing personalized medicine, facilitating the collection and analysis of patient data to inform treatment decisions. As our understanding of the human genome and other biological factors advances, personalized medicine and data management become increasingly crucial in optimizing patient care. "Artificial intelligence has rapidly transformed various industries, including healthcare, by enabling the individualization of treatment based on genetics, environment, and lifestyle factors. "Insurance plays a crucial role in alleviating the financial burden of healthcare, promoting private healthcare usage, and easing pressure on public healthcare systems. Moreover, it indirectly contributes to government revenue, especially when foreign insurance is utilized. However, challenges arise when insurers exert control over reimbursements, potentially impacting patient care by dictating treatment protocols and limiting choice of care providers," says Datuk Dr Kuljit. He elaborated by explaining that restricting access to certain healthcare providers within insurance networks can impede patients' freedom to choose their preferred care settings. Sometimes, insurance policies, despite appearing comprehensive, may contain hidden limitations that hinder coverage when needed. Embracing innovation in insurance methodologies and leveraging data-driven models can enhance coverage certainty and ensure adequate financial protection in times of illness, even amidst rising healthcare costs. “Insurance coverage mitigates reliance on public healthcare facilities, thereby indirectly bolstering government revenue through hospital visits covered by insurance policies. Utilizing foreign insurance further injects foreign funds into the local economy. However, challenges arise as insurers attempt to manage escalating healthcare costs by imposing restrictions on treatment options and provider choices. While such measures may be necessary to control expenses, they can limit patient access to care and disrupt established healthcare networks. Balancing cost containment with quality care provision remains a complex task for insurers amid the current healthcare landscape. Using technology to calculate the real cost of healthcare Innovative models leveraging AI for reimbursements entail the digitalization of healthcare expense data, enabling precise mapping of the costs associated with each treatment. This includes factoring in the actual profits required by both hospitals and insurance companies, alongside exploring co-payment options and government initiatives for healthcare financing. “Healthcare costs are soaring, making it challenging to raise premiums significantly. This underscores the need for innovative approaches. Currently, healthcare cost calculations are manual and imprecise. To address this, we must leverage technology to accurately calculate expenses. Digitalizing healthcare expenditure data is crucial for analyzing past spending and mapping the precise costs of treatments, considering variables such as patient complications, age, and timeliness of treatment. “Determining the actual profits necessary for both hospitals and insurance companies is essential. Profitability ensures sustainability for all stakeholders. However, this must be done systematically to ensure fairness in premiums and payouts for insurers and hospitals alike. Additionally, exploring co-payment schemes is vital. “Traditionally, insurance coverage has often led patients to perceive healthcare as 'free' upon card swiping. However, given the escalating costs, co-payments from patients are becoming increasingly necessary. Government intervention in healthcare financing is also crucial," Datuk Dr Kuljit added. Collaboration between the government, insurers, and healthcare providers is vital for ensuring equitable access to healthcare while managing costs. Leveraging technology will be key to achieving these goals, paving the way for reinvestment in healthcare. In conclusion, the innovative journey ahead centers on value-based healthcare with frameworks that demonstrate value to patients, payers, and the government. By integrating telehealth, integrated care, personalized medicine, and innovative reimbursement models, we can sustainably enhance healthcare delivery. Innovative healthcare models, such as the virtual ward concept seen in institutions like The North West Anglia NHS Foundation Trust, demonstrate the potential to deliver care remotely.
