​Lipedema: A painful fat disorder affecting extremities

​​Lipedema, also known as lipohyperplasia dolorosa, is a chronic, painful disorder characterized by the abnormal accumulation of fat, primarily in the extremities like the legs and arms.

First identified in 1940 by Dr Allen and Dr Hines, this condition disproportionately affects women and can cause significant physical and emotional distress. Lipedema is often triggered by hormonal changes, such as puberty, pregnancy, or menopause, with 80% of cases linked to a family predisposition. 


"The legs are swollen, bruise easily, and feel tender or painful," explains Dr Martin Barsch, Aesthetic Dermatologist and  Head of Center for Lipedema Austria and Aesthetic-Vein-Laser Practice, Austria. He presented his findings on the use of liposuction under tumescence local anesthesia for lipedema patients at the recent Aesthetic Medicine & Surgery Conference & Exhibition (AMSC).

Dr Martin describes lipedema as feeling like "bumps inside fat," with its onset commonly during hormonal shifts. Patients often experience symptoms such as heaviness in the legs, fatigue, and pressure pain, where even the lightest touch can cause discomfort. The condition leads to the disproportionate distribution of fat, particularly in the legs, giving them a column-like appearance, while the upper body remains relatively thin.

Lipedema stages
Lipedema progresses through four stages:
  • Stage 1: Smooth and soft skin with hypodermal enlargement.
  • Stage 2: Irregularities with the formation of pearl-sized nodules.
  • Stage 3: Deforming fat masses and tissue hardening, resulting in the two-body syndrome (thin upper body, thick legs).
  • Stage 4: Concomitant lymphedema, known as lipolymphedema.

In most cases, the fat distribution is symmetrical. However, there are exceptions when other conditions, such as lymphedema or venous insufficiency, are present, leading to uneven fat buildup. Diagnosis often involves a combination of clinical observation and diagnostic imaging, such as ultrasound, which helps rule out other conditions like edema.

Treatment options
For patients with lipedema, lifestyle modifications such as sports and dietary changes are often recommended. Compression garments and lymphatic drainage can provide some relief, particularly for those with concomitant lymphedema. However, for advanced cases, liposuction offers the most effective treatment.

Dr Martin emphasizes that tumescence local anesthesia (TLA) is the preferred method for liposuction in lipedema patients. This technique involves infusing a low-concentration solution into the tissues to allow for smooth fat removal with minimal blood loss and reduced postoperative pain. Water-assisted liposuction, performed under shallow anesthesia or epidural, is also used.

Energy-based devices like radiofrequency or cryotherapy are typically avoided in lipedema treatment due to the large surface areas involved. "Tumescence liposuction is highly effective, with the ability to remove large volumes of fat while sparing the lymphatic system," says Dr Martin. 

He warns against exceeding the safe lidocaine dosage during the procedure, citing a 2019 case where a patient suffered a fatal cardiac arrest after receiving more than 70 mg/kg of body weight in lidocaine. The international consensus limits the dose to 55 mg/kg.

Lipedema vs. aesthetic liposuction
Lipedema liposuction differs from cosmetic liposuction, as it focuses on pain relief rather than aesthetics. Lipedema patients are often older, with comorbidities, and require multiple surgeries (typically two to four). Aesthetic liposuction, on the other hand, targets smaller areas with the goal of achieving a visually pleasing result.

For older patients, liposuction may need to be combined with skin resection to address loose skin. Younger patients generally experience better skin retraction following liposuction.

Dr Martin shared his experience from 3,000 liposuction procedures, noting that only two male patients were treated, one of whom had Klinefelter syndrome. The average age of lipedema patients was 38, with a mean body mass index (BMI) of 30, indicating that half of the patients were obese. However, BMI is not a reliable indicator of fat distribution in lipedema.

Complications and postoperative care
While liposuction is generally safe, high-volume procedures can lead to complications, such as circulatory dysregulation, nerve damage, or secondary lymphedema. Patients are advised not to stay home alone on the day of surgery, as they may experience dizziness or other side effects.

Despite the risks, liposuction significantly improves the quality of life for lipedema patients. "Lymph-sparing liposuction is the only curative treatment for lipedema," Dr Martin concludes. "It drastically reduces pain and enhances mobility, allowing patients to regain control of their lives."
