​Liposuction: Key considerations before choosing the procedure

​​Liposuction can be an effective way to reshape and contour the body, but it’s important to understand the procedure’s complexities and risks. It’s not just about removing fat—there are various factors that determine a successful outcome, especially when combined with other procedures like abdominoplasty.

Dr Ananda Dorai (pix), Consultant Plastic, Reconstructive Cosmetic and Burn Surgeon at Thomson Hospital, Kota Damansara, shared insights on lipo-abdominoplasty at the Aesthetic Medicine & Surgery Conference & Exhibition (AMSC) on the crucial elements to consider before undergoing liposuction.

The basics
By definition, brachioplasty can be performed on its own or in combination with liposuction. When these two procedures are combined, the results are often more pronounced. However, a deep understanding of the vascular anatomy, particularly of the abdomen is essential for safe planning.

“The abdominal area, in particular, requires meticulous attention to vascular structures,” Dr Ananda notes. “Liposuction is not a simple weight-loss solution but rather a body contouring procedure. Patients must be fully aware of this distinction.”

Patient selection is key
A critical aspect of liposuction is selecting the right candidates. “As with any cosmetic surgery, the selection criteria are vital,” explains Dr Ananda. Factors like a patient’s body mass index (BMI), whether they are underweight, overweight, or in obesity class 1, are taken into consideration. Additionally, patients who have had multiple pregnancies, significant weight changes, or excess abdominal skin and fat should be carefully evaluated.

The procedure also involves assessing the laxity of the anterior abdominal wall and the myoaponeurotic system, which affects the results. Understanding a patient’s expectations is crucial for a successful outcome.

Liposuction is not for everyone
Dr Ananda highlights that not everyone is a good candidate for simultaneous liposuction and abdominoplasty. "For example, those with a large mandibular dermal hernia face a high risk of injury to the abdominal structures," he explains. In such cases, separating the procedures or opting for alternative solutions may be safer.

Pre-surgical assessments: What you need to know
Patients over the age of 40 or those with specific risk factors should undergo a detailed pre-surgical evaluation. This often includes a full cardiac screening, an echocardiogram, a stress test, and lung function tests, especially if the patient is a smoker or has been one in the past. Routine blood tests and imaging such as an abdominal or pelvic ultrasound are also necessary.

"Some additional precautions include stopping oral contraceptives or vitamin supplements at least one to two weeks before the surgery,” Dr Anand advises. “We also check for personal history of venous thrombosis, pulmonary embolism, and any previous abdominal scars.”

The surgical process
The surgery itself can last anywhere from four to six hours, depending on the surgeon’s approach. The patient is required to stand or sit for pre-operative markings, which ensure the areas targeted for liposuction are well-defined.

Liposuction typically involves a water-assisted technique, particularly with Polytech, to ensure smoother fat removal. This method also allows fat harvesting and transfer to other parts of the body, such as the breasts.

For those undergoing abdominoplasty with liposuction, Dr Ananda emphasizes the importance of preserving the abdominal flap's blood supply. “The selective upper lining technique is invaluable, as it spares the intercostal and subcostal perforators, which in turn maintain the flap’s vascular integrity,” he explains.

Post-operative care
Closure techniques during the procedure are designed to minimize complications. “We use two layers of closure for rectus plication,” says Dr Ananda, “and progressive tension suturing to reduce the risk of seroma formation.”

Patients should be aware that recovery can be lengthy, and complications such as fluid accumulation (seroma) or blood clots are possible. Proper follow-up care is essential for a smooth recovery.

Liposuction, particularly when combined with other procedures like abdominoplasty, is a versatile and effective body contouring technique. However, its success depends on strict case selection, a thorough understanding of patient anatomy, and realistic expectations.

For anyone considering liposuction, consulting with a board-certified surgeon and understanding all aspects of the procedure—from pre-surgical assessments to post-operative care—is key to achieving the best possible results.
