Sensitive skin care: Breaking down skin barrier repair and allergy management

​​Sensitive skin is a common concern that affects millions of people worldwide, leading to discomfort, irritation, and other dermatological issues. Addressing sensitive skin requires understanding its causes, including medical conditions and allergic reactions. Moreover, managing sensitive skin involves repairing the skin’s barrier and regulating its microbiome. This article explores various treatment strategies, including moisturizers, emollients, and targeted skincare ingredients to reduce inflammation, as well as medical causes that contribute to sensitive skin.

Moisturizers for skin barrier repair
A key approach to managing sensitive skin is repairing the skin barrier. The stratum corneum, the outermost layer of the skin, plays a crucial role in maintaining skin hydration. When this barrier is compromised, the skin becomes dry, irritated, and more susceptible to environmental damage. Moisturizers are essential for enhancing the hydration capacity of the skin, making them a primary tool in managing sensitive skin.

Ingredients such as panthenol, polyols, beta-glucan, and PCA-Na are beneficial for increasing skin moisture. These components work to lock in hydration and prevent water loss from the skin. Furthermore, emollients with water-retaining effects, like petrolatum, lanolin, plant oils, squalene, and paraffin, help in creating a protective layer on the skin, preventing moisture evaporation. For more in-depth repair, structural lipids like ceramides, fatty acids, and cholesterol are vital for repairing the intercellular lipid layer, restoring the skin’s barrier function.

Microbiome regulation in sensitive skin
Recent studies have highlighted the importance of regulating the skin’s microbiome in managing sensitive skin conditions. The microbiome, composed of trillions of microorganisms living on the skin’s surface, plays a pivotal role in skin health. Products containing prebiotics, postbiotics, or probiotics can effectively regulate the microbiota, maintaining a healthy balance and improving skin resilience. This is particularly important for sensitive skin, which may react negatively to changes in the microbiome.

In addition to microbiome regulation, ingredients targeting neuroinflammation are beneficial. Compounds like Trans-4-t-Butylcyclohexanol and Acetyl dipeptide-1, which inhibit the capsaicin receptor (TRPV1 ion channel), help relieve inflammation and reduce skin sensitivity. According to Dr Wong Siu Bee (pix), Consultant Dermatologist at Ting Skin Specialist Clinic, hydroxysafflor yellow A, resveratrol, ginger root extract, evening primrose oil, and oatmeal-derived anthranilic acid are particularly effective in inhibiting inflammatory factors and providing relief for sensitive skin. These insights were shared during Dr Siu Bee's presentation at the Aesthetic Medicine & Surgery Conference & Exhibition (AMSC).

Medical causes of sensitive skin
Sensitive skin can be attributed to several medical conditions. Common causes include irritant contact dermatitis, allergic contact dermatitis, contact urticaria, rosacea, physical urticaria, dermographism, aquagenic pruritus, dry skin, eczema, and photodermatoses. Conditions like cutaneous mastocytosis and carcinoid syndrome also contribute to increased skin sensitivity. Understanding the underlying medical cause is crucial for effective treatment.

Understanding allergic reactions
The term "allergy" comes from the Greek word allos, meaning reaction. An allergy is an abnormal and harmful immune response to typically harmless substances, known as allergens. There are four types of allergic reactions, classified by Gell & Coombs in 1963:

Type 1 – IgE-mediated hypersensitivity: Commonly known as immediate hypersensitivity, this reaction involves allergens like pollen, pet dander, or certain foods. IgE antibodies bind to mast cells, leading to the release of histamines and other mediators, which cause symptoms such as hives, eczema, or even anaphylaxis in severe cases.

Type 2 – IgG-mediated cytotoxic hypersensitivity: This occurs when antibodies bind to cells, leading to their destruction. An example includes transfusion reactions or hemolytic disease of the newborn.

Type 3 – Immune complex-mediated hypersensitivity: Immune complexes form and deposit in tissues, triggering an inflammatory response. Diseases like rheumatoid arthritis and systemic lupus erythematosus are typical examples.

Type 4 – Cell-mediated hypersensitivity: This is a delayed response, with reactions often appearing 12 to 48 hours after exposure. Common manifestations include contact dermatitis caused by allergens such as nickel, fragrances, or plants.

Diagnosing and managing allergic reactions
Allergic reactions, especially type 1 hypersensitivity, can present as life-threatening conditions, such as anaphylaxis, which requires immediate medical attention. According to Dr Siu Bee, tests such as the radioallergosorbent test (RAST) can detect IgE antibodies in the blood, while skin prick testing is more specific and faster for identifying potential allergens.

Treatment for anaphylaxis includes adrenaline, corticosteroids, and antihistamines. For non-life-threatening allergic reactions, managing symptoms through avoidance of allergens and using antihistamines for mild symptoms is common. Patients prone to severe reactions may be prescribed EpiPens for emergency use.

For type 4 hypersensitivity, such as allergic contact dermatitis, avoiding the causative agent is essential. A patch test is often used to confirm the allergen, with emollients and topical steroids providing symptom relief. In severe cases, systemic corticosteroids or immunosuppressive agents may be prescribed.

Distinguishing sensitive skin from allergic reactions
Accurately distinguishing between sensitive skin and allergic reactions is crucial for effective treatment. Sensitive skin often results from environmental factors, product use, or underlying skin conditions, while allergic reactions are triggered by specific allergens. Managing sensitive skin requires gentle skincare practices, such as using moisturizers and avoiding harsh chemicals, while managing allergic reactions involves identifying allergens and reducing inflammation.

As Dr Siu Bee emphasized, "Accurate diagnosis is the key to effective management, helping patients lead a more comfortable life by addressing both sensitive skin issues and allergic reactions.
