​​"Your skin barrier is key to maintaining overall skin health and preventing long-term damage," explains Dr E Chia Qi (pix), a dermatologist from Hospital Sungai Buloh, during her talk, ‘Skin Care – Friend or Foe’ at the Aesthetic Medicine & Surgery Conference & Exhibition (AMSC). Dr Qi stresses that a well-tailored skincare routine is vital not just for aesthetic appeal, but also for maintaining skin integrity, preventing damage, and boosting confidence. 

Skin care has exploded in popularity, with thousands of new products hitting the market, leaving many overwhelmed by what to choose. Each person's skin care routine should be unique to them, as it varies depending on skin type, lifestyle, and specific skin concerns.

The science of skin: A brief overview
The skin’s outermost layer, the stratum corneum, acts as a critical barrier. Imagine it as a brick wall: corneocytes are the bricks, and intercellular lipids (made up of ceramides, cholesterol, and fatty acids) form the mortar that holds it all together. These lipids play a significant role in maintaining hydration and skin protection. Tiny molecules, known as the natural moisturizing factor (NMF), also live within the corneocytes, ensuring the skin barrier remains intact and functions effectively.

The stratum corneum serves five primary functions:
  1. Barrier function to protect against harmful agents.
  2. Homeostasis to maintain internal moisture.
  3. Photoprotection from harmful UV rays.
  4. Antimicrobial defense against bacteria.
  5. Host to skin microbiota, which contributes to overall skin health.

10 Myths about skincare (and the truth behind them)
Myth 1: "All sunscreens work the same way."
Reality: Sunscreens come in two types—chemical (like oxybenzone or avobenzone, which absorb UV rays) and mineral (such as zinc oxide or titanium dioxide, which physically block UV rays). Mineral sunscreens are less likely to cause allergic reactions but may leave a white residue.

Myth 2: "Sunscreen causes cancer."
Reality: Without sunscreen, you're more at risk of skin cancer due to harmful UV radiation. The benefits of using sunscreen far outweigh any potential risks.

Myth 3: "I have dark skin, so I don’t need sunscreen."
Reality: Darker skin tones may not burn as easily but are still vulnerable to UV damage and hyperpigmentation, making sunscreen essential.

Myth 4: "My sunscreen is waterproof, so I don’t need to reapply after swimming or sweating."
Reality: There is no such thing as "waterproof" sunscreen. Sunscreens may be water-resistant, but they need to be reapplied after water exposure.

Myth 5: "SPF50 means I don’t need to reapply."
Reality: SPF50 provides slightly more protection than SPF30, but both still need reapplication every two hours or after water exposure.

Myth 6: "My makeup contains SPF, so I don’t need extra sunscreen."
Reality: Unless your makeup contains at least SPF 30, you will need to apply a dedicated sunscreen layer.

Myth 7: "I only need sunscreen when it's sunny."
Reality: UV radiation can penetrate clouds, and UVA rays (which contribute to aging) are present year-round, making sunscreen a daily necessity.

Myth 8: "I can rely on oral sun-protection supplements."
Reality: Supplements may provide additional protection but are not a substitute for topical sunscreen.

Myth 9: "I won’t get enough vitamin D if I wear sunscreen."
Reality: Studies show that regular sunscreen use does not significantly reduce vitamin D levels. UVB rays trigger vitamin D production, but everyday sunscreen use does not lead to a deficiency.

Myth 10: "Sunscreen never expires."
Reality: Sunscreen does have an expiration date, and using expired products can leave your skin unprotected. Always check the date before use.

Starting a skincare routine: Know your skin type
Understanding your skin type is the first and most important step in building an effective skincare routine. Skin can be categorized into four key characteristics:
  1. Oily or dry
  2. Sensitive or resistant
  3. Pigmented or non-pigmented
  4. Wrinkle-prone or tight

These elements result in 16 unique skin types, with the ideal being oily, resistant, non-pigmented, and tight. Depending on your skin type, here are some general guidelines:
  • Oily skin: Opt for foaming cleansers and lightweight moisturizers (or skip moisturizer if your skin doesn’t need it).
  • Dry skin: Use non-foaming, lipid-sparing cleansers and barrier-repair moisturizers.
  • Sensitive skin: Stick with hypoallergenic products containing anti-inflammatory ingredients like aloe vera, niacinamide, or colloidal oatmeal.
  • Pigmented skin: Prioritize sunscreen, tyrosinase inhibitors, and antioxidants.
  • Wrinkle-prone skin: Use sunscreen daily, along with retinoids and antioxidants to minimize signs of aging.

How to build a basic skincare routine
For beginners, it’s important to keep things simple and focus on consistency, not the number of products. Here's a basic routine:
  1. Step 1: Cleansing
    A gentle cleanser removes dirt and excess oil without irritating the skin. For most people, cleansing twice a day with lukewarm water is sufficient.
  2. Step 2: Moisturizing
    Moisturizers help restore and strengthen the skin barrier. Choose one that suits your skin type, whether it's oil-free for oily skin or a rich, ceramide-packed moisturizer for dry skin.
  3. Step 3: Sun protection
    UV light is the main culprit behind premature aging and skin cancer. Use a broad-spectrum sunscreen with at least SPF 30 and reapply every two hours.

What to look for in skincare products
When choosing products, pay close attention to active ingredients, concentrations, and claims. Look for products that contain clinically proven ingredients like:
  • SPF and antioxidants (for anti-aging)
  • Hyaluronic acid and vitamin E (for dryness)
  • Kojic acid, AHAs, and vitamin C (for pigmentation)
  • Retinoids and benzoyl peroxide (for acne)
  • Azelaic acid (for rosacea)

Don’t fall for marketing hype
Expensive doesn’t always mean effective. Natural ingredients aren’t automatically better, either—people can be allergic to natural compounds just as easily as synthetic ones. A skincare routine should be personalized and effective, not determined by marketing trends.

Evening skincare routine: The importance of night care
While a morning routine focuses on protection, your evening routine is all about repair. A common approach includes:
  • Double cleansing (if wearing makeup or sunscreen)
  • Exfoliating (once or twice a week with AHAs or BHAs)
  • Retinoids (to reduce fine lines and wrinkles)
  • Moisturizing (using a heavier night cream with glycerin or hyaluronic acid)

Night creams, designed for deeper hydration, may contain active ingredients like retinol to improve skin texture and reduce the appearance of wrinkles.

Beyond the basics: Toners, serums, and add-ons
Toners, though not essential, can help balance pH and remove impurities. There are three main types of toners:
  1. Hydrating: Adds moisture to the skin.
  2. Astringent: Controls oil and tightens pores.
  3. Exfoliating: Removes dead skin cells and helps with skin renewal.
Adding serums (like vitamin C or hyaluronic acid) to your routine can further address skin concerns such as hyperpigmentation or dehydration. Be mindful of product combinations, though—mixing certain acids (like AHAs) with retinol can cause irritation.

Keeping your skincare routine simple and effective
Consistency is the key to healthy skin. Start with a basic routine that includes cleansing, moisturizing, and sun protection. Over time, as your skin's needs become more apparent, you can incorporate advanced treatments like retinoids, serums, and exfoliators. The skin is a dynamic organ that requires constant care and attention. Don’t be swayed by skincare myths or marketing gimmicks—find what works for you, and always consult a dermatologist if you’re unsure.
