​Revolutionizing skin tightening: The rise of needling radiofrequency

​​​​​​In recent years, the demand for nonsurgical skin tightening procedures with minimal downtime has skyrocketed, increasing by a staggering 600% in the past 15 years. Treatments that deliver effective results without the risks and recovery time of surgery are in constant demand.

Radiofrequency (RF) technology has been a game changer in this field, and by 2021, RF-based non-surgical skin tightening treatments accounted for approximately 35% of the market. Among the various types of RF technology, needling radiofrequency (NRF) is emerging as a leader in both efficacy and satisfaction.

Dr Patrick Huang (pix), a renowned dermatologist and the Executive Director of the Taiwanese Dermatological Association, highlighted the groundbreaking impact of NRF during the Aesthetic Medicine & Surgery Conference & Exhibition (AMSC). His presentation, titled "Revolutionizing skin rejuvenation and scar revision with needling radiofrequency," provided deep insights into how NRF is transforming skin rejuvenation treatments and beyond.

The emergence of needling radiofrequency:
Monopolar RF (MRF) was the first RF-based device approved for cosmetic use in 2002. While effective, early RF technologies had limitations, including the risk of epidermal damage and the inability to control heating depth with precision. This led to the development of needling radiofrequency (NRF) in 2009, which improved upon earlier methods by using microneedles to deliver heat directly to the dermal layers, avoiding damage to the skin surface.

“NRF allows us to precisely control the temperature and regulate the pulse width, resulting in better collagen remodeling and reduced risk of epidermal damage,” explained Dr Patrick. This technology offers a non-invasive approach to tighten deep dermal structures while minimizing discomfort and downtime.

Why NRF is superior
One of the key advantages of NRF is its ability to deliver uniform heat to the dermal layers while preserving the epidermis. By doing so, it initiates a wound healing response that promotes long-term dermal remodeling. Studies have shown that newly formed collagen fibers replace damaged zones as early as three months post-treatment. By the 10th week, the treated area is consistently regenerating, showing increased collagen production and a smoother, more youthful skin appearance.

Additionally, NRF stimulates both neocollagenesis and neoelastogenesis, contributing to firmer, more elastic skin. In clinical trials, those who underwent NRF treatment reported a 25% to 43% improvement in fine lines and wrinkles, particularly around the eyes, after just three sessions. The effects were not only noticeable but also long-lasting, with improvements visible even six months after treatment.

Needling vs stamping
A common question is why needling is preferred over stamping when delivering RF energy. While both methods induce tissue reactions, needling offers more control over the depth and duration of RF energy exposure. This precise regulation results in better collagen remodeling and improved skin texture. Dr Patrick noted that needling allows for “deeper penetration without damaging the skin surface,” making it an ideal solution for patients seeking subtle but significant improvements in skin smoothness and contour.

Impact of RF on fat tissue
One of the lesser-known benefits of RF technology is its effect on fat tissue. RF-induced apoptosis, or controlled cell death, causes changes in fat cells that lead to a gradual reduction in fat volume. Studies show that RF treatments reduce fat cell size and alter cell morphology, promoting the safe elimination of triglycerides through natural bodily processes. Unlike other fat-reduction procedures, RF-induced apoptosis does not cause inflammation or necrosis, making it a safer and more controlled option for fat reduction.

According to Dr Patrick, “Patients experience a noticeable improvement in skin smoothness and contour after two NRF treatments. Fat reduction occurs gradually, allowing for more natural-looking results without dramatic changes to the skin surface.”

Heat management and patient comfort
One of the main concerns with any thermal-based treatment is comfort. NRF treatments often incorporate external cooling systems to manage heat, reduce discomfort, and minimize redness and swelling post-treatment. Cooling the skin during treatment enhances the overall experience, ensuring that the energy targets deeper tissues without overheating the skin surface.

Dr Patrick emphasized the importance of effective heat management, noting that it not only improves comfort but also enhances treatment efficacy. “By lowering the skin’s surface temperature, we can increase skin impedance, ensuring the energy is focused on deeper layers for better results,” he explained.

NRF for acne scars
Beyond skin tightening, NRF has proven to be a valuable tool for treating acne scars. Studies involving 175 participants have shown that NRF is safe and effective for various types of acne scars, particularly boxcar and rolling scars. These scars, which are typically U-shaped or M-shaped, respond well to NRF treatments, with significant improvements observed after the first session.

Compared to other treatments like Er - lasers, NRF is more tolerable and has fewer side effects, such as post-inflammatory hyperpigmentation (PIH). This makes NRF a particularly good option for patients with darker skin tones who are more prone to pigmentation issues.

One of the most impressive aspects of NRF treatments is their long-lasting results. In a study of 30 participants with acne scars, patients reported continued improvement in skin texture and scar volume six months after their final treatment. This durability makes NRF a cost-effective solution for patients seeking sustained improvement without the need for frequent follow-up treatments.

Dr Patrick recommends NRF for anyone with atrophic acne scars or darker skin types, as it offers a safe, effective, and minimally invasive treatment option. Additionally, NRF can be combined with other treatments, such as fractional lasers, to enhance its efficacy without increasing the risk of adverse effects.

As the demand for nonsurgical skin tightening continues to rise, NRF is leading the charge as one of the most effective and versatile treatment options available. From skin rejuvenation and tightening to acne scar reduction, NRF offers long-lasting results with minimal downtime and discomfort. With further advancements in RF technology, the future of skin tightening and rejuvenation looks brighter than ever.
