Facial erythema, or facial redness, can have various causes ranging from mild and transient to chronic and pathological. While a natural blush may appear cute and healthy, persistent facial erythema can cause significant discomfort and social anxiety, often accompanied by itching, burning, or stinging sensations.

The psychosocial toll of persistent facial redness
Dr Leong Kin Fon (pix), a pediatric dermatologist at General Hospital Kuala Lumpur, presented findings on the psychosocial impact of chronic facial conditions at the recent Aesthetic Medicine & Surgery Conference & Exhibition (AMSC). Referencing the study "The Psychosocial Impact of Chronic Facial Dermatoses in Adults," Dr Leong discussed how the research, which included 166 acne patients, 134 rosacea patients, and 120 seborrheic dermatitis patients, revealed that these conditions can severely affect one’s quality of life.

Dr Leong noted that patients with rosacea scored highest on the Dermatology Life Quality Index (DLQI) and Social Appearance Anxiety Scale (SAAS), showing a high prevalence of anxiety. “Seborrheic dermatitis patients showed the highest rate of depression,” Dr Leong remarked. Interestingly, while DLQI, HADS (Hospital Anxiety and Depression Scale), and SAAS results correlated moderately, their relationship to disease duration and severity appeared weak, suggesting that even mild cases may cause considerable distress.

Rosacea: More than just a red face
Rosacea, a common chronic condition, presents various symptoms, including papules, pustules, visible blood vessels, eye issues, and persistent redness. These symptoms often result from neurogenic inflammation rather than histamine, making standard antihistamine treatments largely ineffective.

According to data from the National Skin Center (NSC) in Singapore, rosacea affects a diverse range of patients, refuting the misconception that it primarily affects Caucasians. Out of over 1,000 cases reviewed, nearly half showed no identifiable triggers, indicating that rosacea can emerge unexpectedly.

Causes and types of facial erythema
Facial erythema can arise from multiple skin conditions, including dual-pathology cases where rosacea coexists with conditions like seborrheic dermatitis. Diagnosing erythema requires close examination to distinguish early (primary) stages from later (secondary) developments. Key signs of rosacea may include thickening skin, papules, pustules, and even reactions to spicy foods or alcohol, often mistaken for acne.

“Accurate diagnosis involves observing lesion surfaces at various angles,” Dr Leong advised. Erythema from rosacea typically lacks scaling, which differentiates it from conditions like eczema. However, patients may sometimes experience “two-in-one” cases, with rosacea on one side of the face and seborrheic dermatitis on the other.

Diagnosing facial erythema: Surface and margins matter
Identifying whether erythema is due to rosacea or eczema requires checking for unique lesion characteristics. Some lesions may be ill-defined, while others are well-defined, offering clues for diagnosis. "Look closely," Dr Leong emphasized, “even slight differences in color—whether red or purplish—provide diagnostic insights.”

Modern treatments for facial erythema
Recent advances in treatment, including non-steroidal options like Crisaborole, offer more effective relief for facial erythema. Combination therapies can help manage both rosacea and seborrheic dermatitis symptoms. However, treating chronic erythema is a gradual process, requiring months rather than days to show noticeable improvements.

Managing flare-ups and long-term treatment
Properly managing facial erythema can lead to significant improvement, although flare-ups may persist, particularly for chronic conditions like rosacea and eczema. Eczema, for instance, often undergoes dynamic changes over months or even years. If left untreated, it can lead to post-inflammatory hyperpigmentation, underscoring the importance of early intervention.
