​​Terpenes are a diverse class of organic compounds produced by plants and some insects. Known for their aromatic properties, terpenes have become a subject of significant interest in various fields, including perfumery, medicine, and the cannabis industry. This article delves into the fascinating world of terpenes, exploring their structure, roles in nature, applications, and potential therapeutic benefits.

Understanding terpenes: The basics
Terpenes are natural products with the formula (C5H8)n, based on an isoprene unit. They are categorized into different types based on the number of isoprene units they contain. Two isoprene units form monoterpenes, three units form sesquiterpenes, and larger numbers form carotenoids. Terpenoids are modified terpenes with additional functional groups.

With around 55,000 identified terpenes and terpenoids in the plant kingdom, these compounds play a crucial role in various biological processes. They are involved in plant defense against herbivores and diseases, attracting pollinators, and facilitating plant-plant communication.

Terpenes in everyday products
Terpenes are widely used in numerous products that we encounter daily. They are key ingredients in perfumes, cosmetics, cleaning products, and food flavorings. Additionally, they serve as solvents (e.g., turpentine) and pesticides.

The widespread use of terpenes is a testament to their versatility and safety. In the UK, for instance, terpenes are classified as simple food supplements, which means they do not require extensive regulation.

Terpenes in cannabis: An overview
Terpenes are volatile compounds, meaning they can easily evaporate, especially during the extraction process from cannabis plants. Traditional extraction methods that use heat can cause the loss of these valuable components. To preserve terpenes, cold extraction methods, such as cold pressing, are employed. However, even with these methods, some terpenes can be lost if the cannabis is irradiated to meet microbiological standards.

The characteristic smell of cannabis is primarily due to its terpene content, not its cannabinoids. Beyond their aromatic contributions, terpenes in cannabis have significant medical properties. Although research is still in its early stages, terpenes are believed to enhance the effects of major cannabinoids like CBD and THC, potentially boosting their medical benefits.

The medical properties of terpenes
Myrcene
Myrcene, one of the most common terpenes in cannabis, smells like hops, which are genetically related to cannabis. Known for its analgesic properties, myrcene is a potent painkiller. It also has anti-diabetic properties, stabilizes blood sugar levels, and acts as an antioxidant, potentially playing a role in long-term neurological diseases like multiple sclerosis.

Moreover, myrcene is strongly anti-inflammatory, making it beneficial for conditions like inflammatory bowel disease. It is also sedative, antibacterial, and has shown anti-cancer properties in vitro. Additionally, myrcene improves the permeability of the blood-brain barrier, enhancing the absorption and efficacy of THC and CBD.

Limonene
Limonene, characterized by its citrus scent, is another significant terpene in cannabis. It has a range of therapeutic properties, including anti-anxiety, anti-inflammatory, antioxidant, analgesic, anti-fungal, antibacterial, and anti-cancer effects. Limonene also possesses antidepressant properties, making it potentially useful in treating various mental health conditions.

Pinene
Pinene, as its name suggests, has a pine-like aroma. It exists in two forms: alpha-pinene and beta-pinene. This terpene offers numerous benefits, including analgesic, anti-anxiety, antibacterial, antibiotic, anti-inflammatory, and antioxidant properties. Pinene also has neuroprotective effects, making it valuable for cognitive health.

Linalool
Linalool, commonly found in lavender, is known for its calming effects. It has anti-anxiety, sedative, anti-inflammatory, and analgesic properties. Linalool is particularly useful in enhancing the anti-convulsant properties of CBD, making it beneficial for epilepsy treatment.

Beta-caryophyllene
Beta-caryophyllene, present in cloves, is unique among terpenes because it directly interacts with the endocannabinoid system. It is anti-inflammatory, antioxidant, a gastric protector, and an analgesic. Its interaction with the endocannabinoid system enhances its potential therapeutic applications.

Terpenes and cannabis strains
The specific terpene profile of a cannabis strain significantly influences its effects. For instance, two strains with 20% THC can have markedly different effects due to their terpene and flavonoid profiles. Understanding these profiles is crucial for determining the medical efficacy of a particular strain.

Professor Dr Mike Barnes (pix), a Consultant Neurologist from the UK, highlighted the importance of terpenes in botanical medicine during the 5th International Integrative Medicine Conference 2024. He emphasized that terpenes not only contribute to the therapeutic effects of cannabis but also help tailor the treatment to specific conditions and patient needs.

Flavonoids: The overlooked compounds
Flavonoids, another important component of cannabis, are often underreported. These polyphenolic compounds are found throughout the plant kingdom and are responsible for the color and smell of flowers. They play a role in attracting pollinators, protecting plants from UV radiation, and defending against microbial attacks.

In cannabis, about 26 flavonoids have been identified, contributing approximately 2.5% of the dry weight of the plant, similar to terpenes. Flavonoids have various therapeutic properties, including anti-inflammatory, antioxidant, anti-cancer, and neuroprotective effects.

The entourage effect
The combined effects of cannabinoids, terpenes, and flavonoids create what is known as the entourage effect. This phenomenon suggests that the whole plant is more effective than its isolated components. For example, full-spectrum cannabis products, which include all these compounds, are often more effective than products containing only isolated cannabinoids.

The future of terpene research
Despite their potential, the study of terpenes in human medicine is still in its early stages. More research is needed to fully understand their therapeutic benefits and how they interact with other compounds in cannabis. Professor Dr Mike Barnes advocates for greater transparency from cannabis producers, urging them to include terpene and flavonoid profiles in their product analyses to aid in medical prescriptions.

Terpenes are versatile compounds with significant roles in nature and potential therapeutic benefits. Their presence in cannabis not only contributes to its distinctive aroma but also enhances its medical properties. As research progresses, the full potential of terpenes in medicine may be unlocked, offering new avenues for treatment and improving the efficacy of existing therapies.
