​​​​The renaissance of psychedelics: From ancient rituals to modern medicine

​​The term "psychedelic," derived from the Greek for "mind manifesting," was first proposed by Humphry Osmond in 1956 to describe the drugs' profound effects on human consciousness. Psychedelics have been used by humans for centuries, if not millennia, with Western scientific research beginning in the late nineteenth century with the discovery of mescaline and continuing with the development of LSD in the 1940s.

"Psychedelics played an important role in the development of biological psychiatry in the 1950s," said Professor Dr Vicknasingam (pix), Professor of Addiction Medicine at the Center for Drug Research, USM UNODC, WHO. Speaking at the 5th International Integrative Medicine Conference 2024, themed "Healthspan - The Science and Art of Wellbeing," organized by the Association of Integrative Medicine Malaysia (AIMM), he noted that psychedelic-based therapies were explored until the late 1960s when LSD was banned.

​"During that period, LSD and magic mushrooms were prevalent in the market. American soldiers brought these cultures to Southeast Asia, where misuse of these substances began. However, over the last 20 years, there has been renewed interest in studying psychedelics for their therapeutic value."

In the United States, many startup companies are heavily investing in psychedelic research with the hope of bringing these products to market. But what is the current regulatory framework for psychedelic medications globally?

In North and South America, psychedelics have historical uses, with religious exemptions in the US and decriminalization in parts of South America. In Europe, psychedelics are mostly decriminalized, while in Australia and Canada, they are approved for medical use. Thailand is the only Asian country to have approved psychedelics for medical use, though it may be reconsidering this decision. The acceptance and regulation of psychedelics are largely seen in the Western world.

Professor David Nutt, in a review published by The Guardian, argues that the risks of psychedelics have been grossly exaggerated and their benefits minimized. He advocates for the British government to reschedule psychedelics to facilitate scientific research. Nutt's argument highlights the difficulty of conducting research on substances under international control, which complicates regulatory approvals and patient recruitment.

Psilocybin and depression: Promising results
A systematic review and meta-analysis published in the British Medical Journal this year examined the efficacy of psilocybin for treating depression. The results indicated significant improvements in patients with secondary depression, suggesting positive outcomes. Another review, "Psychedelic Drugs: Neurobiology and Potential for Treatment of Psychiatric Disorders," published in 2020, found that changes in self-experience, emotional processing, and social cognition might contribute to the therapeutic effects of psychedelics.

The literature review "Psychedelics for Patients with Cancer" assessed the role of psychedelics in treating anxiety and depression among cancer patients. The evidence suggested that only a few sessions could provide long-lasting benefits, though the studies had methodological limitations, including increased blood pressure and heart rate. Despite these challenges, psychedelics offer new hope for patients inadequately treated with conventional medications.

Safety and efficacy in mental health disorders
Another recent systematic review and meta-analysis, "Psychedelic Therapy for Depressive Symptoms," found a significant reduction in depressive symptoms associated with psychedelic therapy. However, the small number of studies and low sample sizes necessitate careful interpretation of these results. Similarly, the review "Efficacy and Safety of Psychedelics for the Treatment of Mental Disorders" focused on safety, showing that psychedelics are well-tolerated with low risk of serious adverse events in controlled settings.

Challenges in conducting rigorous research
Over the last 20 years, there has been a surge in interest in understanding psychedelics and their therapeutic potential. From thousands of studies conducted, only a handful have been well-designed due to the challenges of conducting rigorous clinical trials under strict regulatory controls. Despite these hurdles, psychedelics have shown promise in treating PTSD, treatment-resistant depression, anxiety, depression related to cancer, and substance use disorders. MDMA and psilocybin have received FDA breakthrough designations for treating PTSD and treatment-resistant depression, enabling fast-track regulatory review.

The emergence of numerous startup companies in the United States reflects the growing interest in psychedelic research, driven by the hope of bringing these treatments to market. However, as Professor Nutt and other experts argue, scientific evidence must be robust, and research must be free from political pressures and regulatory hurdles to realize the full potential of psychedelics.

The path forward: Embracing psychedelics in modern medicine
The challenge of conducting randomized clinical trials for psychedelics is compounded by the need for blinding, as the perceptible effects of these substances make it difficult to create a placebo. Additionally, public funding is essential for longer-term, phase three controlled clinical trials to produce reliable results. Most current trials have small sample sizes, making it difficult to draw strong conclusions.

The future of psychedelics in medicine hinges on larger, well-designed, randomized controlled trials and a deeper understanding of their chemistry and neurobiology. While early results are promising, the data remain preliminary, requiring validation and replication. The potential of psychedelics to revolutionize mental health treatment is vast, but it will only be realized through rigorous scientific inquiry and regulatory support.
