A common misconception about aesthetic medicine is that it's solely about making someone look beautiful. Everyone possesses their own unique beauty. Aesthetic medicine is fundamentally about preserving and enhancing this natural beauty while minimizing complications. It’s a delicate balance that requires expertise and care. To embark on a career in aesthetic medicine, one must understand the comprehensive process of starting, managing, and ultimately exiting the practice.

The foundation of aesthetic practice
In Malaysia, the Letter of Credentialing & Privileging (LCP) is a significant credential for medical doctors and dental surgeons. "To begin practicing aesthetic medicine, one must first have a solid medical foundation, followed by business acumen and medico-legal knowledge," explains Professor Dr Ramamurthy Subramaniam (pix), Head of Department at the International Institute of Wellness & Aesthetic Medicine, Malaysia. He shared these insights at the Association of Integrative Medicine Malaysia (AIMM)’s 5th International Integrative Medicine Conference 2024.

Why choose aesthetic medicine?
The allure of aesthetic medicine lies in its lucrative and glamorous appeal. The global aesthetic medical market, valued at $15.4 billion in 2023, is projected to reach $25.9 billion by 2028. With a compounded annual growth rate of 11%, the Asia-Pacific region is leading this surge due to increased healthcare expenditure, improved infrastructure, and growing patient awareness.

Defining aesthetic medicine
Aesthetic medicine encompasses a multidisciplinary approach carried out by registered medical and dental practitioners. It focuses on evidence-based treatments aimed at fulfilling aesthetic desires without complications. This field involves both non-invasive and minimally invasive procedures, requiring specialized training and competency exams like the LCP.

The importance of anatomy
"A solid understanding of anatomy is crucial in aesthetics," notes Professor Dr Ramamurthy Subramaniam. This includes knowledge of skin, muscles, ligaments, blood supply, and nerve supply. Practitioners must understand how facial anatomy changes with age to achieve harmonious and balanced results.

The aging process
Understanding the natural aging process is essential in aesthetic medicine. Factors such as fat displacement, bone resorption, muscle activity, and skin changes must be considered. This knowledge helps practitioners perform procedures that enhance natural beauty without creating an artificial look.

Popular aesthetic procedures
Aesthetic medicine offers a wide range of procedures beyond Botox, such as Dysport, Xeomin, and the newer Daxxify, which lasts longer than traditional Botox. Dermal fillers, both hyaluronic acid-based and synthetic, are also popular for their varying benefits and reversibility.

Skin rejuvenation
Skin rejuvenation techniques, including non-engineered and engineered skin boosters, are revolutionizing the field. Platelet-rich plasma (PRP) and barbed sutures are also gaining popularity for their effectiveness in tissue regeneration and skin tightening.

The role of lasers
Lasers are a cornerstone of aesthetic treatments, capable of achieving results that toxins and fillers cannot. From skin tightening to hair removal, lasers offer a versatile range of applications with advancements continually emerging.

Body contouring
Body contouring and fat reduction are significant areas of interest in aesthetic medicine. Techniques like cryolipolysis and injectables help address localized fat and improve body aesthetics.

Complications in aesthetic medicine
Aesthetic medicine is not without risks. Professor Dr Ramamurthy highlights cases where complications arose due to unlicensed practitioners or improper procedures. Understanding the potential complications and how to avoid them is crucial for any aesthetic practitioner.

Business operations in aesthetic medicine
Running an aesthetic practice requires a strong business foundation. "You need to plan your start, operation, development, and exit strategy," advises Professor Dr Ramamurthy. This involves careful planning of budget, capital expenses, site selection, and licensing.

Renovation and human resources
Overspending on renovations can hinder cash flow, while managing human resources effectively ensures a smooth operation. Practitioners must balance staffing needs without overextending financially.

Marketing and communication
Effective marketing and communication are vital for a successful aesthetic practice. Budgeting, operating expenses, and understanding key business metrics are essential for growth and sustainability.

Planning for exit
"Plan your exit from the beginning," emphasizes Professor Dr Ramamurthy. Exiting at the peak of success, rather than when exhausted or struggling, ensures a smoother transition. Business planning, policies, and processes play critical roles in a successful exit strategy.

Aesthetic medicine is a complex field that requires a blend of medical knowledge, business acumen, and an understanding of human anatomy and aging. It's not just about making someone look beautiful but preserving and enhancing their natural beauty safely and effectively. As the market continues to grow, practitioners must stay informed, cautious, and prepared for the challenges ahead.
