Advances in endocrine surgery as treatment for cancer

​​​​​​​​Surgery continues to play a pivotal role in managing endocrine cancers, particularly in cases where the disease is localized. Despite the availability of chemotherapy, radiation therapy, hormone therapy, and targeted therapies, surgical intervention remains the cornerstone of curative treatment for endocrine malignancies.

“Endocrine surgery is a specialized field aimed at treating cancers of the thyroid gland, parathyroid gland, adrenal gland, and neuroendocrine tumors affecting the pituitary, pancreas, and gastrointestinal tract,” explained Dr Nurul Nadia Jamaludin (pictured), Breast and Endocrine Surgeon at Aurelius Hospital, Negeri Sembilan, during her presentation at the National Cancer Congress Malaysia.

Thyroid cancer: A leading endocrine cancer in women
The Malaysia National Cancer Registry (until 2016) highlights breast and colorectal cancers as the most common cancers in the country. Although endocrine tumors do not rank among the top ten overall, thyroid cancer is the eighth most common cancer in Malaysian women.

Endocrine surgeries for thyroid cancer often address well-differentiated thyroid carcinomas, including papillary, follicular, and medullary thyroid carcinomas. Rarely, cases of undifferentiated anaplastic thyroid carcinoma are surgically treated when diagnosed early. Additionally, surgeries for parathyroid and adrenal cancers, such as adrenocortical carcinoma, malignant pheochromocytomas, and paragangliomas, are undertaken based on the functionality of the tumor.

Surgical goals: Curative and palliative approaches
The primary objectives of endocrine cancer surgery are improving survival, reducing recurrence risk, and enabling precise disease staging. Surgical intent can be categorized as curative—to completely remove the cancer—or palliative, which aims to alleviate symptoms, improve quality of life, and manage tumor burden in advanced cases.

Factors influencing the surgical approach include:
  • Patient-related factors: Age, overall health, and performance status.
  • Cancer-specific factors: Type, stage, and location of the tumor.

Dr Nurul emphasized, “The intent and scope of surgery should be tailored to the patient’s needs, disease factors, and overall treatment goals.”

Case insights: Complex thyroid cancer surgeries
Dr Nurul shared challenging cases to illustrate the complexities of endocrine cancer surgery: A female patient presented with a large thyroid nodule pushing the trachea out of alignment, with significant vascular involvement. Another patient had a thyroid tumor extending into the mediastinum, requiring a multidisciplinary team, including a cardiothoracic surgeon, for successful resection. “These cases underscore the importance of personalized surgical planning and multidisciplinary collaboration,” she noted.

Technological evolution in endocrine surgery
Advancements in imaging and surgical techniques have transformed endocrine cancer care. Traditional open surgeries are being replaced by minimally invasive approaches, such as laparoscopic and robotic-assisted surgeries.

Key benefits of minimally invasive techniques include:
  • Reduced surgical morbidity.
  • Faster recovery times.
  • Improved cosmetic outcomes.

Procedures like retroperitoneal laparoscopic adrenalectomy and robotic adrenalectomy have become standard in many centers worldwide. “In selected cases, minimally invasive thyroid lobectomy is now recommended for low-risk, well-differentiated thyroid cancers,” added Dr Nurul.

Challenges in endocrine surgeries
Despite advancements, endocrine cancer surgeries remain complex and demanding. Key challenges include:
  • Patient age and pre-existing conditions.
  • Cancer type, stage, and extent of spread.
  • Need for multidisciplinary expertise in advanced cases.

Palliative surgeries: An essential option
For some patients, palliative surgeries, such as adrenalectomy, are invaluable in improving quality of life and disease control. Dr Nurul highlighted the importance of selecting appropriate cases for such interventions to ensure meaningful benefits.

A multidisciplinary approach: The key to success
Endocrine cancer treatment thrives on a multidisciplinary approach, combining the expertise of surgeons, oncologists, radiologists, and other specialists. Personalizing treatment plans to align with patient-specific needs and disease characteristics is critical for optimizing outcomes and improving survival.
