​Can you be overweight and still have low blood pressure?

When we think about weight and blood pressure, we often picture a direct link: gaining weight leads to high blood pressure, while being slim means lower blood pressure. But is it really that simple? Can someone be overweight and still have low blood pressure? Let’s delve into this topic and uncover the truths behind these common misconceptions.

While it’s true that being overweight is generally associated with high blood pressure, it’s entirely possible for someone who is overweight to experience low blood pressure as well. In fact, shedding some pounds can often help lower blood pressure and enhance overall heart health—even if you currently have low readings.

Datuk Dr Sanjiv Joshi (pix), a consultant cardiologist at Pantai Hospital Kuala Lumpur, sheds light on this issue. He notes, “Persons on the larger side can also experience low blood pressure. They may feel dizzy, weak, and fatigued, often struggling to find energy. Many come in with readings as low as 90/50 or 100/50, which can be genetic.”

It’s important to understand that being obese doesn’t automatically mean you’ll have high blood pressure. “Many people present with low blood pressure, regardless of their weight,” Dr Sanjiv explains. “There’s a tendency to develop hypertension, but it’s not a strict rule. If blood pressure drops too low, steroids might be prescribed, but we usually recommend increasing salt intake and avoiding quick position changes—from sitting to standing, for example.”

Typically, overweight individuals have a higher risk of developing high blood pressure due to increased blood volume and vascular resistance linked to excess body fat. However, when someone loses weight, their body becomes more efficient at circulating blood, often resulting in lower blood pressure.

So, why might some overweight individuals have low blood pressure? Here are a few possible reasons:
  • Fat distribution: The way fat is distributed in the body matters. Those with more fat in their legs might experience lower blood pressure compared to those with more abdominal fat.
  • Underlying medical conditions: Conditions like hypothyroidism can lead to fatigue and low blood pressure, which may affect overweight individuals.
  • Medication side effects: Some medications aimed at managing weight or other health issues can lower blood pressure as a side effect.

What to do if you’re overweight and have low blood pressure
If you find yourself in this situation, here are some lifestyle modifications to consider:
  • Gradual weight loss: Aim for a slow and sustainable weight loss plan. Quick fixes often lead to setbacks.
  • Stay hydrated: Drinking plenty of fluids helps maintain adequate blood volume, which is crucial for healthy blood pressure.
  • Diet adjustments: Focus on regular meals that include sufficient protein and, if necessary, sodium to help manage blood pressure.
  • Exercise regularly: Engage in moderate exercise to boost your overall health, but be cautious of potential dizziness.
